DirectorCareAtHomeServicesleadsthedevelopmentandexecutionofaHealthdeliverysystemwithin COPEHealth Solutions and CHS Medical Group, focusing on improving the health outcomes ofa specific group of individuals. This role involves leveraging data analytics, understanding the healthcare landscape, and collaborating with various stakeholders to implement targeted interventions and programs.  

KeyResponsibilities:  

• DevelopandimplementaCaredeliverystrategythatalignswiththeorganization'sgoalsand objectives. 
• Usedataanalyticstoidentifytrends,patterns,andopportunitiesforimprovementwithinthe population. 
• Designandimplementtargetedinterventionsandprogramstoaddressspecifichealthneeds and improve outcomes. 
• Collaboratewithvariousstakeholders,includingphysicians,nurses,caremanagers,andpayers, to ensure effective implementation of the strategy. 
• Monitortheperformanceofthepopulationhealthstrategyandmakesadjustmentsasneeded. 
• Worktoimprovethequalityandcostofcareforthepopulation. 
• Engagewithexternalstakeholders,suchascommunityorganizationsandregulatoryagencies,to promote the care delivery strategy and health outcomes. 
• Mentoranddeveloptalentwithintheorganizationtosupportthepopulationhealthstrategy. 
• Understandsandmanagesthefinancialimplicationsofthepopulationhealthstrategy. 
• Ensurescompliancewithrelevantregulationsandpolicies. 
• DevelopclinicalandoperationalprocessesfortheCHSMedicalGroupandtheCareatHome Solutions 
• Dyad with CMO and EVP of CHS Medical in developing, implementing, and maintaining standardsandtrainingfortheCHSMedicalGroupandCareatHomeSolutionsdivision,including provision of technical quality assurance requirements.  

RequiredSkills&Qualifications :

• Leadership: Strong leadership skills and the ability to motivate and engage a team.  
• Communication: Excellent communication and interpersonal skills to effectively collaborate with diverse stakeholders. 
• Analytical: Strong analytical skills to interpret data and identify trends.  
• Strategic Thinking: Ability to develop and implement strategic plans.  
• Project Management: Strong project management skills to oversee the implementation of programs and initiatives.  
• Knowledge of Healthcare: In-depth knowledge of the healthcare industry, including managed care, value-based care, Medicare and Medicaid in California

Qualifications 

• 7-11+yearsofworkhealthcareadministrationexperience,inadditiontheidealcandidatewill have at least 4+ years of health care consulting experience 
• Clinicaldegree(e.g.,PA,NP,RN,LCSW,PhD) 
• Master’sdegreeinanapplicablefield(MBA,MSN,MPH,MHAorsimilar)stronglypreferred. 
• Abilitytotravelupto 80%. 
• Flexibilitytoworkeveningsandweekendsasnecessary. 
• Bilingual- Spanish speaking is a requirement
