Job Description

Primary Job Responsibilities:

  1. Lead the day-to-day functions of the unlicensed nursing staff, including directing and correcting work.
  2. Supervise resident care specialist on shift in documentation and work assignments.
  3. Have direct responsibility for coordination and delivery of residents nursing needs.
  4. Provide hands-on care to residents as directed.
  5. Receives Physician Orders and communicates with pharmacies to ensure prescribed medications are properly set up and available to the resident.
  6. Act as liaison between the physician, family, and other departments in the community.
  7. Foster self-respect and a feeling of worth in each resident by consistent kindness, understanding, and patience in their care.
  8. Attend and participate in continuing education programs designed to keep you abreast of changes in the profession.
  9. Consistently work cooperatively with residents, staff, volunteers, and families to ensure residents are receiving the best quality care.
  10. Assist in orientation for new employees to their duties according to the facility guidelines.
  11. Organize and maintain all records necessary, following facility policies and procedures.
  12. Participate in the Nursing on-call rotation [Full-Time: Mandatory / Part-time: On-Request]
  13. Follow all written and oral directions given by supervisor.
  14. Participate in all in-services and activities as required.
  15. Use proper nursing skills and techniques to ensure safety of self, residents, volunteers, visitors, and staff.
  16. Observe all facility policies and procedures, including but not limited to safety, infection control, residents’ rights, and those contained in the employee handbook.
  17. Maintain the confidentiality of all protected health information whether electronic, written, or oral exposed to either during the course of assigned duties or as a result of an incidental disclosure.
  18. Provides episodic and intermittent health promotion or disease prevention counseling and teaching self-care in meeting routine and special health care needs that can be done by other staff under the supervision of the registered nurse.
  19. Performs required documentation.
    1. Completes with DON quarterly evaluation of resident.
    2. Completes requested nursing assessments as directed.

Required Qualifications:

  1. Pass background check before and during employment
  2. Graduate from an approved school of nursing.
  3. Active CPR certification
  4. Currently licensed in Illinois as an LPN or RN.
  5. Able to follow written and oral directions.
  6. Demonstrate dependability, cooperation, and interest in the care of elderly.
  7. Display leadership qualities, good communication skills, and a desire to continuously learn.
  8. Possess the ability to communicate effectively and deal tactfully with personnel, residents, families, visitors, government agencies, and the general public.

Physical and Environmental Demands:

  1. Frequently: Sit, stand, and walk
  2. Frequently: Stoop, push and pull
  3. Occasionally: Squat, balance, kneel, crouch, and reach above shoulders
  4. Occasionally: Lift up to 50 lbs.
  5. Frequently: Lift up to 25 lbs.
  6. Regularly: Use hands for simple and firm grasping and fine manipulation.
  7. Occasionally: Exposure to hazardous chemicals such as disinfectants, cleaners, soap, etc.
  8. Occasionally: Exposure to latex gloves and other personal protective equipment.
  9. Occasionally: Exposure to blood/body fluids as required to provide resident care.
  10. Regularly:    Exposure to microbial bacteria and other infectious agents inherent to care of residents

Wage Range:

LPN: $22.50 – $24.50/hour (Actual wage based on relevant experience)
RN: $26.00 – $28.00/hour (Actual wage based on relevant experience)
